2012 CPS Sports Physicals

Peak Performance and the Columbia Orthopaedic Group will be performing 2012 Sports Physicals for Columbia Public School athletes on Saturday, February 25, starting at 8am. These physicals are open to all Junior and Senior High Athletes. You should get a physical form from your Coach/Athletic Director, or go to the link below. You should have the history and signatures filled out prior to arriving that morning. These physicals are good for summer camps as well as all sports during the 2012-2013 school year!!!
